Hospice patients and their families will receive the following range of home care services:Social work, Spiritual care, Bereavement support, Counseling, Volunteer services, Respite care, Self-care assistance, Therapies,Medical equipment, Medical supplies, Medication management, Palliative care, and Companionship. When end-of-life care centers around pain management, our home-based care will at least ensure peace of mind for terminally ill patients, and a maintenance of the patient’s connection to their family. We will coordinate our care services with hospice professionals, and help these patients to feel comfortable. One of the services we will provide during this important time is support not only for the patient, but for the caregivers in their family, and among their friends. As quality of life is the first priority with hospice care, our home care professionals will support that mission by coordinating services that align seamlessly with the needs and wants of the patient and their loved ones. During the in-home assessment, the family will be able to share the details of the specific situation and make their needs and wants also known to our caring and experienced staff.
