Pre-Hospice Program

Pre-Hospice Program

We will develop a pre-hospice program, which will be designed to give elderly patients the care they want at home and keep them out of the hospital. Our Social Workers and Nurses will regularly visit patients in their homes to explain what they can expect in their final years, help them make end-of-life plans and teach them how to better manage their diseases. Physicians will track their health and scrap unnecessary medications. Unlike hospice care, patients will not need to have a prognosis of six months or less, and they can continue getting curative treatment for their illnesses, not just for symptoms.

Pre-hospice programs provide care and support for elderly people who are facing end-of-life issues. These programs are designed to provide comfort and care for those who are facing a terminal illness or who may be dealing with the physical and emotional impacts of aging. They can be an important resource for families and caregivers who are trying to navigate the difficult decisions that come with end-of-life care.

A pre-hospice program typically includes a team of health care professionals who provide specialized end-of-life care and support. This team may include medical doctors, registered nurses, social workers, spiritual counselors, and hospice aides. The team works together to provide comprehensive care tailored to the individual needs of the elderly person. This care may include physical and emotional support, as well as pain and symptom management.

Pre-hospice programs also provide emotional and spiritual support for the elderly person and their family members. They may assist with difficult conversations about end-of-life care, offer emotional and spiritual guidance, and help families better understand the dying process. In addition, pre-hospice programs may provide access to community resources and support services, such as transportation, home health aides, and respite care.

Pre-hospice programs can be a valuable source of comfort and support for elderly people and their families. They provide specialized care and resources that can help families make informed decisions and cope with the difficult emotions that come with end-of-life care.
